What the inspector wrote

On 10/07/25 Licensing Program Analyst (LPA) Valerie Flores made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA Flores met with Applicant/Administrator, Clarissa Tenonciang, and was granted entry into the facility. LPA Flores conducted a tour of the interior/exterior areas of the facility. On 5/27/25 the Riverside County Fire Department approved (6) six ambulatory residents only. LPA observed the following during today's visit: The home is a two-story structure consisting of (3) three resident bedrooms, (5) five live-in staff/ family members bedrooms, (3.5) three and a half bathrooms, kitchen, formal dining room, great room, laundry room, small living room, garage, and a backyard. The bedrooms were observed to have met the required bedding, furniture, lighting, night stand, dresser, and area for seating. The bathrooms had non-skid mats, and grab bars. LPA observed extra linen (sheets, blankets, towels) to be in good repair stored in a cabinet near the laundry room .One (1) fully charged fire extinguisher was located near the facility entry-way. Facility maintains a (2) two day supply of perishable foods and (7) seven day supply of non-perishable foods. (2) Two additional refrigerators were observed in the garage designated for live-in family and live-in staff. LPA observed a locked cabinet in the kitchen which stores knives and other sharp items. A locked closet was observed in the lower-level passageway for centrally stored medication. The hot water temperature was measured at 108.4 degrees Fahrenheit meeting within the required limits. The garage stored emergency food and water supplies to meet the needs of (6) six anticipated residents. The facility has a pool which is observed to have a locked gate securing all perimeters of the body of water. LPA observed an outdoor patio with a shaded seating area was available for residents. (Continue to LIC809C...) (Continuation from LIC809) The facility has a sufficient supply of dishes, cooking and eating utensils, that were observed to be in good repair. Indoor and outdoor passageways were free of obstruction. Per Applicant/Administrator, Clarissa Tenonciang, facility does not anticipate retaining any firearms and/or ammunition on the premises. The facility has an emergency disaster plan and approved infection control training plan on file. LPA Flores observed the required postings of the facility sketch, personal rights, Administrator Certificate, PUB475 CCLD Complaint poster and the Long term Care Ombudsman poster. During today's visit, LPA Flores did not observe any issues and/or concerns. Applicant is awaiting to attend the COMP III located at the Riverside Regional Office. Final approval of licensure will be determined by the Centralized Application Bureau (CAB). An exit interview was conducted and a copy of this report was provided to Applicant/Administrator, Clarissa Tenonciang.
