What the inspector wrote

On 09/29/2025, Licensing Program Analyst (LPA) Janette Romero made an announced visit at the home for a pre-licensing evaluation. LPA met with Applicants Elva Rodriguez and Berenice Yanez, and Administrator, Charles Parker. On 04/29/2025, an initial application was received by the Centralized Applications Bureau (CAB) to operate a Residential Care Facility for the Elderly in the home. LPA reviewed a signed Fire Safety Inspection Request (STD. 850) dated 07/25/2025, noting the home was granted a fire clearance for one (1) bedridden and five (5) non-ambulatory elderly residents, ages 60 and over. Administrator Parker's administrator's certificate expires on 08/03/2027. LPA toured the facility's interior and exterior with Applicant Rodriguez and Administrator Parker. The facility is made up on a one-story home with three (3) resident bedrooms, two (2) bathrooms, a kitchen, dining room, two (2) living rooms, and attached car garage. Indoor and outdoor passageways are free of obstruction. The facility has operating utilities and there are no bodies of water on the premises. Administrator tested one of the smoke alarms/carbon monoxide detectors and LPA heard them to be operational. LPA also observed a charged fire extinguisher mounted in the kitchen. The facility has a central heating and air conditioning system installed with a central panel located in the hallway to control the entire house. Resident bedrooms are adequately furnished with a bed, chair, drawer/closet space and functional lighting. Bathrooms have grab bars and non-skid mats in the showers. LPA tested the hot water temperature in each one of the restrooms, which initially measured at 140-degrees Fahrenheit. Administrator reported they lowered the water heater temperature. LPA then measured the hot water temperature a second time, which measured at 92.3-degrees F. By the end of the visit, LPA measured the hot water temperature a final time, which measured at 107.6-degrees F. LPA toured the living and dining room and observed the facility has a sufficient seating for the requested capacity. The living room fireplace is also adequately screened. LPA toured the kitchen and observed the facility has storage space for a two-day supply of perishable foods and seven-day supply of non-perishable food items. Knives and sharp instruments will be secured in a kitchen cabinet next to the refrigerator. Disinfectants and cleaning solutions are secured in a locked cabinet under the kitchen sink. Medications will be centrally stored in a locked cabinet near the resident bedrooms. The facility has a confidential storage area designated for staff and resident records. Emergency disaster plan, residents’ personal rights, complaint procedures, facility sketch, and Long-Term Care Ombudsman's information are visibly posted near one (1) of the living rooms. During today's visit, LPA did not observe any issues or concerns. Applicants were advised a licensing program manager will contact them to complete the COMP III requirement at the Riverside Regional Office. Final approval of licensure will be granted by the CAB analyst. An exit interview was conducted, and a copy of this report was reviewed and provided to Applicant Rodriguez.
