Pre-licensing visit·February 18, 2026

Licensing Program Analyst (LPA) Janira Arreola conducted an announced Pre-Licensing visit. LPA met with Applicant, Faiz Khan and Administrator Musarrat Khan who were informed on the purpose of the visit. The applicant is seeking an initial license for an RCFE. The facility is a one story home with (4) bedrooms and (2) bathrooms. There is no pools on the premises no fire arms are being stored at the facility. The local fire department approved the facility for (6) non ambulatory residents one of which may be bedridden. LPA observed the physical plant, fixtures and furniture were present and in good repair for future residents. The kitchen has the ability to prepare food in a clean environment and had cooking and eating utensils for future clients. The facility has the required supply of perishable and non-perishable foods. The facility has required postings. Centrally stored medications, dangerous items, and cleaning supplies have designated locked areas. There is a complete first aide kit. LPA observed linens, hygiene supplies, and PPE supplies for future clients. The facility has charged and inspected fire extinguishers. Smoke and carbon monoxide detectors in working order and interconnected. Hot water temperature was read in the kitchen at 120F. The facility has a land line number at: 951-208-0031. The passage ways are free of obstruction and there is a clear egress path in case of evacuation. There are no immediate health or safety concerns and no objections at this time. An exit interview was conducted and a copy of this report was reviewed and provided.
