Pre-licensing visit·April 7, 2026

On 4/7/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an announced pre-licensing inspection. LPA Flores met with Applicant Kenya Torres and explained the purpose of the visit. LPA Flores conducted a tour of the facility alongside the applicant and observed the following during the visit: The facility is a single-story structure which consisted of (4) four resident bedrooms, (2) two bathrooms, laundry room, living room, den, garage, and backyard. Indoor and outdoor passageways were free from obstruction. LPA observed an outside area that was covered and fully furnished. Resident bedrooms were equipped with the required bedding, furniture, and functional light. Bathrooms were sanitary and odorless. Shower areas and toilets were equipped with grab bars and slip-resistant material. Hallways that led to non-private bathrooms were equipped with night lights. LPA observed a hall closet with equipped with extra linen. The kitchen area was clean and organized. LPA observed a cabinet designated for disinfectants, knives, and other sharp items. LPA's observed a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Carbon monoxide and smoke detectors were tested and were observed to be fully operable. LPA did not any bodies of water during the visit. Water temperature measured within the required limits. LPA observed two fully charged fire extinguisher. Laundry services are conducted on-site. Washer and dryer were observed to be fully operable. Per applicants, there are no firearms and/or ammunition on the premises. LPA observed the posted see something say something, LTCO information, emergency disaster plan, facility sketch, and personal rights. LPA confirmed there is a designated area for staff and resident files. COMP III has not yet been scheduled. LPA has determined the facility meets the operational requirements for licensure . Final approval of licensure will be determined by the Centralized Application Bureau (CAB). The prelicensing inspection is complete and has satisfied all requirements in accordance with Title 22, California Code of Regulations. An exit interview was conducted, and a copy of this report was discussed and provided to Applicant.
