What the inspector wrote

Allegation #3 - R1 interview reveals staff assist with him with escorting to the dinning area for each meal times. R1 also reveals that has tray service is utilized where staff deliver meals upon R1's request. S1 concluded R1's tray service and escorting to and from to the dinning area during meal times. Allegation #4 - Staff failed to assist resident in care pertains to call buttons not being answered. R1 interview reveals that the call button is utilized for service from staff related to incontinence care, transfers, escorting and food services. During time of investigation, call button was tested and found to be in working order. Based on the information obtained there is not enough evidence to support the allegations in this complaint. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. This report was signed by LPA Prieto and Executive Director Marc Pacia.
