Inspector’s narrative

What the inspector wrote

Supporting documentation, including payment ledgers, collection records, and a timeline of the eviction process, was provided for review. R1, R2, and R3 vacated the premises voluntarily before receiving final eviction notices. Similarly, R4 received an eviction notice under the same process and is currently awaiting a final notice to vacate. Eviction records for R4 were documented in the same manner. Based on the findings, there is insufficient evidence to conclude that staff unlawfully evicted the residents while they were under care. As such, the allegation is deemed UNSUBSTANTIATED at this time. This report was signed by LPA Prieto and Executive Director Pacia and a copy was left with the facility.
