Allegation #3 - R1 states that his showering needs are being met. R1 service plan was provide to LPA by S1 indicating that R1 has full assistance with showering. S1 states that R1's needs are being met of being assisting R1 with bathing, 1 person assist, 2X per week. Allegation #4 - R1 states that his dressing needs are being met. R1 service plan was provide to LPA by S1 indicating that R1 has full assistance with dressing. S1 states that R1's needs are being met of being assisting R1 with dressing in the morning and at bedtime. Based on the information obtained there is not enough evidence to support the allegations made in this complaint. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. This report was signed by LPA Prieto and Executive Director Pacia and a copy was left with the facility.
