Mangente at Community Care Licensing Division (CCLD) Adult and Senior Care (ASC) Regional Office for an Informal Meeting. During today’s meeting the following matter was discussed of ownership and control of property: § Twin Hearts Senior Care II: § Twin Hearts Senior Care, LLC § ET Woodville Manor § A copy of Department of Labor § A copy of Corporation Active § Compliance of Annual Fee § Administrator Documents Licensee stated they will provide all copies to Community Care Licensing by 5/28/2025. An exit interview was conducted where this report, LIC809, LIC809D, were discussed and provided to Licensee Kristine A. Mangente. Along with a copy of appeal rights.
