What the inspector wrote

Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced facility visit to conduct a Case Mangement Incident Visit. LPA met with Administrator Karen Roper and explained the purpose of today's visit. The visit is in response to an incident that may have occurred with Staff #1 (S1) and Resident #1 (R1). During today's visit, LPA requested resident roster and staff roster, and staff phone numbers. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report LIC809 was discussed and provided to Administrator Karen Roper.
