What the inspector wrote

Interview with the Administrator reveal R1 was admitted to the facility as an emergency placement by Adult protective services (APS); however, the Administrator and four (4) staff interviewed deny not allowing residents to leave the facility with family. Six (6) residents interviewed deny that staff refused to allow them to leave the facility with family . Based on record review, interviews with Administrator, staff and residents, the allegations are Unsubstantiated. An Unsubstantiated finding means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted with where this report was discussed and a copy provided with appeal rights to Administrative Assistant Gamez at the conclusion on the visit.
