What the inspector wrote

Licensing Program Analyst (LPA) Becky Mann made an unannounced visit to the facility to amend the finding of Complaint Control Number 56-AS-20250812162509. During the facility visit, LPA Mann toured the facility and observed 5 window screens on one side of the building to be in disrepair. LPA Mann informed Theresa Gamez, Assistant Executive Director that deficiency will be issued as this pose potential health, safety and personal rights risks to residents in care. An exit interview was conducted where this report was discussed and provided to Karen Roper, Executive Director
