On 9/30/2025 at 9:30 AM Licensing Program Analyst's Paola Guerrero and Eldin Serrano made unannounced visit to follow up on a substantiated complaint of unlicensed care being provided, 56-AS-20250905152726. On 9/10/2025, the department delivered a substantiated finding that unlicensed care was being provided at 7828 Basset Ct Eastvale, CA 92880 and form LIC 9099 was provided to Operator Shonte Billingsley. Today (9/30/2025), LPA's Guerrero and Serrano conducted a Case Management Visit and conducted a tour of the outside perimeter of the home and found that the home was vacant. On 9/30/2025 LPAs contact the operator who informed LPAs that the home is vacant, and no one is currently available to access the home. The Operator informed LPAs that all tenants moved out of the home and were relocated on 9/22/2025. No immediate health/safety concerns observed at the time of the visit. No civil penalties were issued during the visit. An exit interview was conducted where this report, LIC 809, was discussed and copy was left at the door Per the Operator Shonte Billingsley request.
