Licensing Program Analyst (LPA) Debbie Palacios conducted an unannounced visit to the facility to initiate the investigation into the allegations listed above. LPA met with Executive Director (ED) Cheree Escandel and was informed of the purpose of the visit. LPA was informed by Executive Director (ED) Cheree Escandel that (R1) does not reside at the facility; (R1) is residing at Atria Hacienda. LPA reviewed the Resident roster and (R1) was not listed as a resident. LPA conducted interviews with four (4) residents. This agency has investigated the complaint alleging the above allegations. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. An exit interview was conducted, and a copy of this report was provided.
