On 5/10/21 Licensing Program Analyst (LPA) Shaunte Henry conducted a tele-visit to deliver the findings to the above allegations. The LPA spoke with Administrator Patrick McAdoo-Morton and explained the purpose of the tele-visit. The investigation, which consisted of interviews and file reviews, revealed that the facility did not breech the admission agreement for Resident 1(R1). Documentation and interviews did not indicate that the facility charged R1 for services not provided. The administrator denied charging R1 for services not provided. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time. An exit interview was conducted where this report was discussed with and provided to the administrator via email.
