company. LPA’s kitchen inspection revealed the facility kitchen was free of any pests. LPA inspected all areas of the facility kitchen including under the kitchen sinks. LPA inspected hallways and random residents’ rooms and did not find any pests or any sign of. LPA conducted ten (10) resident interviews and twelve (12) staff interviews. Ten out ten residents interviewed have not seen or heard of any pest issues in the facility. Twelve out twelve staff interviewed have not seen or heard of any pest issues. Based on interviews, observation, and records review, staff did not ensure the facility was free of pests is determined to be Unsubstantiated. A finding of Unsubstantiated means that the allegation may have happened or is valid, but there is no preponderance of evidence to prove that the alleged violation occurred. An exit interview was conducted, and a copy of this report was provided to the Administrator.
