What the inspector wrote

On 1/14/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA met with Administrator Chris Bundalian and LPA explained to Chris the purpose of the visit. A tour of the interior and exterior areas were conducted alongside Chris. LPA observed the following during today's visit: The facility is a single-story structure which consist of (3) three resident bedrooms, (3) staff bedrooms, (3) three bathrooms, office, living room, dining room, kitchen, garage, and backyard area. Facility's targeted population is a residential care facility for the developmentally disabled elderly. The facility provides the residents with laundry services. During the visit, LPA observed the washer and dryer to be fully functioning. The facility maintains social activities like crafts and board games located in the living room to encourage resident interaction. Resident bedrooms were observed to have the required bedding, furniture, seating, and functional lighting. LPA observed a closet which stored additional linen and towels. Indoor/outdoor passageways were kept free from obstruction. Facility maintained a two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Facility maintained emergency food and water in the garage. Knives, cleaning solutions, and other sharp items were stored in a locked cabinet. Medication was observed to be centrally stored in a locked closet in the hallway. LPA observed an additional hall closet which stored an over-supply of client hygiene products. The facility maintained at a comfortable temperature for the residents. LPA observed a covered patio that was fully furnished. The facility's foundation is at the top of a hill; for safety measures, the facility placed a gate around the perimeters of the backyard to ensure resident safety. Per Administrator, there are no firearms and/or ammunition on the premises. (Continue to LIC809C) *(Continuation from LIC809) LPA observed posting of the LTCO information, See Something Say Something, facility license, administrator certification, facility sketch, and personal rights. (4) Four out of (4) four resident records were reviewed. Records reviewed included but not limited to preplacement appraisals, admission agreements, personal rights, care plans, current IPP's, CDER, dental information, and medical assessments. (3) Three out of (3) three staff records were reviewed. Records reviewed included but not limited to health screenings, personnel records, background clearance, fingerprint clearance, employee rights, valid CPR/First-Aid Certification, and relevant training's to assist residents with activities of daily living. An exit interview was conducted and a copy of this report was provided to Administrator.
