Based on LPA Delgado's observation, interview and record review, the licensee did not comply with the section cited above in kitchen area is under construction, permits posted on wall and it was revealed that there were major plumbing issues in the kitchen and corroded pipes need to be repaired. Licensee did not report to CCLD regarding the plumbing issues and the kitchen would need construction which began in May and should be completed by the end of September which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA Delgado's observation, interview and record review, the licensee did not comply with the section cited above in R1 and R2 did not have a current LIC602 which poses/posed a potential health, safety or personal rights risk to persons in care.

Retain original and modified agreements in resident file

Based on LPA Delgado's observation, interview and record review, the licensee did not comply with the section cited above in R1-R5 Admission agreements were requested and unable to be reviewed which poses/posed a potential health, safety or personal rights risk to persons in care.

Mark taps at high-temperature threshold

Based on LPA Delgado's observation, the licensee did not comply with the section cited above in water temperature in two resident bathrooms registered at 127.9 degrees which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain required personnel records for staff

Based on LPA Delgado's observation and record review, the licensee did not comply with the section cited above in records were requested for Administrator current medical training, mandated elder abuse, and S2 Health screening which poses/posed a potential health, safety or personal rights risk to persons in care.

Thisrequirement is not met as evidenced by: Interviews and record reviews revealed S1 took a check from R1's son for payment to the facility and cashed it in personal account and left the country. This poses an immediate health and safety risk to residents in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above due to the discrepancies for Resident One (R1) and Resident Two (R2) MARs which poses/posed a potential health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on observation, interview and record review, the licensee did not comply with the section cited above in having the cleaning chemicals and laundry detergent inaccessible to the residents in care which poses/posed a potential health, safety or personal rights risk to persons in care.
