What the inspector wrote

Licensing Program Analyst (LPA) Jesse Gardner made an unannounced visit to deliver an amended report (18-AS-20211018132918). As part of the visit, LPA toured the facility. During the tour, LPA found the following deficiencies: Call buttons do not work in a resident room No alternatives to the menu Facility was issued two citations per Title 22. An exit interview was conducted where a copy of this report was provided along with copies of the LIC809D, and Appeal Rights.

This requirement was not being met as evidenced by: Based on observation, LPA found that there was not a working auditory device which would alarm staff if a resident were to leave the facility. This poses an immediate health and safety risk to residents in care.

This poses a potential personal rights risk to residents in care.
