What the inspector wrote

On 2/27/2026, Licensing Program Analyst, Arvin Villanueva (LPA), arrived at this facility to conduct the annual inspection visit. LPA met with Administrator, Merly Aguda (AD), and stated the purpose of the visit. Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 12 elderly residents, up to 4 may be non-ambulatory. Facility does manage residents’ cash resources. Facility does not have fire clearance for bedridden, delayed egress, and/or locked interior/exterior. Initial Observation: Present during today's visit were nine residents in care with two staff on duty, S1 and AD. Cameras were observed in the outdoor areas. Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected resident 3 of 6 bedrooms and 2 of 3 bathrooms. All bedrooms are double occupancy. There are three staff rooms. Room temperature was maintained at 72 degrees. The hot water temperature was measured at 123 degrees Fahrenheit. AD called a maintenance person to come and adjust the hot water tank. Fire extinguishers were observed in hallways and were serviced on 3/13/2025. Smoke and carbon monoxide detectors were observed throughout. {1 of 2} In the kitchen, the LPA observed that the facility maintained at least a seven-day supply of non-perishable foods and a two-day supply of perishable foods. The LPA also observed insulin medications stored inside the kitchen refrigerator. The Administrator was instructed to secure these medications in a locked container; however, the Administrator stated that one was not currently available. The facility has an additional refrigerator in the garage. As a temporary measure, the LPA advised the Administrator to place the insulin in the garage refrigerator and secure it using a chain and padlock. The LPA observed the Administrator follow this instruction. Per Administrator, she will obtain a medication box with lock. Two additional freezers were noted in the hallway leading to the garage. The garage also contains another refrigerator and freezer, along with stored chemicals, cleaning supplies, laundry detergents, tools, and miscellaneous items. An advisory was provided to ensure that food items are stored separately from chemicals. Outdoor area was inspected. LPA observed shaded areas and outdoor furniture for residents’ use. Ramps were observed to be in good repair at this time. The facility sits on 5-acre land. LPA observed the grass and garden were maintained. Walkways and ramps were observed to be unobstructed. Based on today's visit, this annual inspection will require a continuation. The Department will return at a later date to continue the annual inspection. Based on observations, deficiencies were observed and citations will be issues on the continuation visit. Exit interview was conducted with the Administrator, and a copy of this report was provided. {2 of 2}
