What the inspector wrote

On April 8, 2025, Licensing Program Analyst (LPA) Kevin Mknelly conducted an unannounced complaint investigation visit to deliver the findings for the above allegations and met with Administrator, Allison Lopez. LPA conducted records review and interviews. LPA is unable to find and or meet the preponderance, per policy. The information provided by the complainant regarding the alleged incident lacked specific time or alleged staff involved. LPA was unable to interview R1 as R1 no longer residend at the facility and R1 lacked the capacity to report historical events. Administrator reported that this alleged incident was discussed some time ago and there was not enough information to investigate further. There were no known witnesses to such an incident. As a result of this investigation, LPA finds allegation to be (US)Unsubstantiated - A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Exit interview with administrator.
