What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual Inspection and met with caregiver staff, Marva Grant, and Administrator, Tatiana Savenco. LPA observed (2) residents present in the common area and (4) residents in their rooms. The facility is licensed for (6) non-ambulatory residents and is approved for (6) residents. Currently (1) resident is on hospice. LPA and staff toured the interior/exterior of the facility including the common areas, (6) private resident bedrooms, (5) resident bathrooms, staff rooms, kitchen, and laundry area. LPA observed the facility to be clean, in good repair and odor-free and each bathroom has the necessary grab bars, non-skid flooring, hygiene items and a trash can. Sharps are locked in the kitchen, toxins are locked in the laundry area, and medications are secured in a separate cabinet. The inside temperature measured 70*F and the hot water measured 108*F in the kitchen. There is 2+ day perishable, including fresh produce, and 7+ day non-perishable supply of food. Smoke/monoxide alarms are working and the fire extinguisher was last serviced 4/21/2025. There are non-audio camera in the common areas and alarms on all exit doors. Outside, there is a patio table with chairs and a cover and three open, unlocked exits and no pools. There are various puzzles, activities and newspapers in the common area for residents to enjoy. LPA reviewed (3) resident files and (2) staff files. Files were found to be organized and current. Medications were reviewed for (1) resident. Orders matched medications being administered and documentation is maintained. All staff are cleared/associated, have completed the required training and have current First Aid/CPR certifications. Administrator's certificate is pending renewal. An updated Dementia care plan and Addendum will be added to the Admission Agreement to reflect recent regulation changes. Updated contact information and insurance information was obtained. There are no citations issued in this report. Exit interview. Copy of report provided to the Administrator.
