What the inspector wrote

Licensing Program Analyst (LPA) Talwinder Bains arrived on 01/28/26 to conduct the annual inspection. LPA met with staff, Benjamin Suiugan and explained the purpose of today's visit. During today's visit , LPA found out that staff, S1 and S2 were working at the facility without fingerprint clearance and not associated with facility. Furthermore, staff, S3 was fingerprint cleared but Not associated with the facility. Based on this information, Deficiencies were observed and cited per Title 22, CCR Regulations 87355 as listed on LIC 809-D. Immediate civil penalties of $1500.00 were assessed today due to staff were working at facility without fingerprint clearance and/or without fingerprint transfer as required. Civil penalties shall be assessed if facility does not comply with POC requirements which were issued today. Exit interview conducted. Copy of this report and appeal rights were provided.
