What the inspector wrote

Licensing Program Analyst (LPA) Victoria Brown arrived unannounced on 6/22/2021 at 11:00am to conclude the complaint investigation. LPA met with Frances Santillan and discussed the purpose of the visit. Regarding allegation " Staff mismanaged resident's medication ", LPA interviewed staff and reviewed the MARs and medication prescriptions during this visit. The investigation revealed that there was a medication error for resident #1 (R1). R1 missed a dose of Insulin named "Lantus" because the staff was unable to locate the insulin. The Administrator Jo Franklin and Nurse Yvonne Holms submitted an incident report to Community Care Licensing (CCL) on 10/9/2020 indicating the medication error and in-service training of staff. Based on interviews and documentation review the preponderance of evidence standards has been met; therefore, the above allegation(s) is found to be SUBSTANTIATED. A finding that the complaint allegation is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Per California Code of Regulations (CCRs) - Title 22, Division 6, Chapter 8, the following deficiencies are being cited on the attached 9099D during this visit. If any of the cited deficiencies are not corrected by the noted due dates; civil penalties may be assessed. The Administrator was provided a copy of their rights (LIC9058) and their signature on this form acknowledges receipt of these rights. This requirement is not met as evidenced by: LPA observed that R1 missed a dose of insulin. Based on confirmation from S1 who stated the insulin was not able to be located until the next morning.This violation poses an immediate health, and safety risk to residents in care.
