On 12/6/2021 at 9:00AM, Licensing Program Analyst (LPA) Chris Hopkins arrived at this facility unannounced to conduct a case management visit regarding three incident reports which were submitted to the Department on 10/05/2021 and 10/07/2021. LPA met with Executive Director (ED) Mike Talani and explained the purpose of the visit. Executive Director Mike Talani stated that the Internal Investigation has concluded, and that there is video evidence of a staff brought in through a staffing resource agency deemed to be the culprit. The Sheriff's Department has taken over this investigation. Executive Director has conducted staff training regarding theft prevention and loss. When admitted, residents are offered to list their valuables. LPA requested copies of the investigation by Sheriff's Department. No deficiencies cited as a result of today's visit. An exit interview was conducted with Executive Director Mike Talani and a copy of this report was provided.
