What the inspector wrote

On September 29, 2025, a virtual office meeting was conducted via Teams, with Licensing Program Manager, Maribeth Senty and Licensing Program Analysts, Kevin Mknelly and Cassie Yang, was conducted with Administrator, Kevin Lee. Also in attendance was Maria Cucicea. The purpose of the meeting is to discuss the pending change of ownership. Items discussed and agreed to include: · The licensee shall provide written notice to the department and to each resident or his or her legal representative of the licensee's intent to change the use of the facility to a new licensee. · If the property is already sold and the transfer of the property has already been completed the licensee shall submit a lease agreement between the property owner and the current licensee (which may have a termination date of when/if a new license is granted). · Licensee will submit a designation of new administrator if applicable as well as a copy of the new administrator’s administrator certificate. · A new administrator or any new staff shall be on the current licensee’s staff roster. This can/ must be done by submitting a transfer request form of through Guardian. · Licensee will submit LIC 500s for the three homes under changes of ownership which include expected hours that the administrator will be present at each facility. · As the licensee you will need to provide CCL with copies of the 60 days' notices that were served to the residents and their responsible parties. Continue LIC 809-C LIC 809-C · The licensee agrees licenses are not transferable; therefore, the buyer of a facility must apply for and obtain a new license from the Department before the ownership of the facility can be transferred. · The licensee is still accountable for the facility until the licensure has been approved. · As a reminder, licenses are not transferable; therefore, the buyer of a facility must apply for and obtain a new license from the Department before the ownership of the facility can be transferred. Documents requested will be submitted to the assigned LPA by Wednesday, Oct 1, 2025. No deficiencies are cited at this time. Report sent via email to licensee for signature.
