What the inspector wrote

On November 13, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with staff and explained the purpose of the visit. There are six residents in care with one resident on hospice services. Today's visit, LPA and staff conducted a tour together to ensure the health and safety of residents in care. Areas toured included but not limited to: resident private bedrooms, bathrooms, laundry room, garage, medication room, kitchen and the common areas. LPA observed resident rooms to be clean and free of dangerous items. LPA observed medication room to be locked and secured. LPA observed pantry to have seven days of nonperishable foods. LPA observed garage to have separate storage for toxins and foods. LPA observed at least two days of perishable foods stored in the garage refrigerator. LPA observed fire extinguisher present with recent service date. LPA and staff did not inspect the upstairs area as residents are prohibited from entering. LPA and staff discussed ensuring the fire doors remain closed at all times. File review was conducted for resident records and personnel records. Based on file review of resident records, it revealed resident (R1) has been living at the facility since 2022 but there are documentation of R1 being bedridden which is out of facility's licensure. CARE tool was completed and deficiencies was cited. Please see LIC 809-D. Exit interview conducted and a copy of the report and appeal rights was provided.

    Based on file review of R1's LIC 601 and LIC 625, the licensee did not comply with the section cited above as facility is licensed for six non-ambulatory but R1's file indicated R1 is bedridden which poses an immediate health, safety or personal rights risk to persons in care.
