What the inspector wrote

On 09/17/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a Required 1 year annual inspection. LPA met with Administrator Gracia Palafox Lapid and explained the purpose of the visit. LPA and Administrator conducted a tour of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen and common areas. While conducting a tour of the kitchen area LPA observed around twenty (20) or so medications to be out on the kitchen table.
