What the inspector wrote

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to continue investigation into allegation listed above. LPA met with Administrator/Licensee Vera Okyere during today's inspection. LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: masks. LPA arrived to continue investigation into complaint from the previous license. During medication review LPA observed 4 of 4 residents were missing medications when compared to physician orders. Administrator stated she does not have documentation or discontinue orders to reflect medication changes. Deficiencies are cited on 809-D. Exit interview and appeal rights provided.
