Licensing Program Analyst (LPA) Christina Valerio arrived to the facility to conduct an annual required inspection. LPA Valerio met with facility staff, and explained the purpose of the visit. LPA Valerio was later met by Licensee Haykush Harutunyan and Qiao 'Vicky' Jiao. LPA Valerio observed two Caregiver staff on shift. LPA Valerio and facility staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms to be fully furnished, free from odors, and clean. LPA Valerio observed residents sleeping in their room, being visited by family, and conducting a doctor appointment with assistance with staff. Resident bathrooms were observed to be clean, fully stocked with hygiene supply, and sanitary. The faucet was measured to deliver hot water within the regulatory range of 105.0 - 120.0 degrees. The facility was observed to have an adequate food supply. The facility thermostat was set to a comfortable temperature. All emergency exits were free from obstructions. LPA Valerio observed two (2) resident files and two (2) staff files. Files were observed to be up to date with required annual documentation. LPA Valerio also reviewed the death report that was submitted to the Regional Office on 02/18/2026. No health or safety concerns observed. LPA Valerio requested the following be sent to CCLASCPSacramentoSouthRO@dss.ca.gov: LIC 500, LIC 308, LIC 610, and copy of Liability Insurance Per California Code of Regulations (CCR) Title 22 - no deficiencies were cited today. An exit interview was held, and a copy of this report was provided.
