Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to the facility to conduct an annual required inspection. LPA Valerio met with facility staff Antoniette Enlope and Ariel Alcantara, and explained the purpose of the visit. LPA was later met by administrator Vidan Barias. LPA Valerio and staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed the common areas of the facility to be fully furnished with no obstructions to emergency exits. Resident bedrooms were observed to be fully furnished with a bed, night stand, chair, closet space, and a light. LPA did not observe any odors coming from the rooms. Resident bathrooms were equipped with a skid matt, chair, hand rails, hand soap, toilet paper and paper towels. The kitchen was observed to have working appliances. The facility was observed to have an adequate food supply along with an emergency supply of food. LPA observed a sign on the fridge that stated residents cannot open the fridge. LPA requested clarification on if the residents have access to food at all times. According to staff report, residents are able to ask for food items after dinner if they wish and staff do not restrict residents from obtaining additional food items. The facility has a large locked cabinet located in the kitchen for sharps and medications. There was a separate closet for toxins, such as cleaning supplies. The fire extinguishers were fully charged and last serviced on Feb 19, 2026. Residents were observed in the bedroom watching television, in the living room, or out on an outing. Staff were observed cleaning, completing administrative task, and doing facility laundry. The exterior plant was observed. LPA observed signal system on exit doors to be turned off. Technical Advisory was provided to staff to ensure the signal system was turned on at all times. All fence panels were observed to be in good condition. There were no obstructions or blockage to exit ways. Continues on LIC 809 - C... LPA Valerio observed resident and staff files. Files were observed to be up to date with annual documentation. LPA requested the following annual documentation be sent to cclascpsacramentosouthro@dss.ca.gov: LIC 500 LIC 308 LIC 610 Copy of Liability Insurance Per California Code of Regulation (CCR) Title 22, no deficiencies are being cited today. A Technical Advisory was provided for the facility's signal system on the exit doorways. An exit interview was held, and a copy of this report was provided.
