What the inspector wrote

LPA Albert Johnson made an unannounced POC visit to the facility to verify correction of citations issued during the case management visit conducted on 10/31/2022. LPA toured the facility, reviewed document submitted for plans of correction observed that the deficiency cited have been cleared. Deficiency cited under Title 22 Regulations have been cleared. Licensee complied with the terms of the POC by POC due date. Facility was provided POC cleared letter. During the POC visit LPA reviewed the chart for R1 and observed a staging of R1's pressure injury as a stage 3 on 11/14/2022. The facility has not request an exception for this and as a result will be given a citation on the attached 809D page. Exit interview conducted,appeal rights given and a copy of this report was left at the facility.

This posed a health and safety risk to resident’s incare.
