On 12/23/2025 Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced visit to the facility. The purpose of this visit was to check resident's medications. LPA met with the facility administrator Christopher Sinon. The current census is 4 with 3 facility staff. LPA entered the facility, and checked resident Medication Administrator Record (MAR) for 3 out of 4 residents and it was complete. LPA checked resident PRN medication record for 3 out of 4 residents in it was complete. LPA spoke with the facility licensee Lit Tien regarding changes that the facility has planned to implement regarding medication/ PRN records, and additional training for facility staff on medications administration and record keeping. Licensee stated that the changes the facility plans to implement regarding medication administrator has to be reviewed and approved by Alta Regional. There were no citations issued during today's visit. A copy of this report LIC 809 was provided to the facility.
