Based on observation, the licensee did not comply with the section cited above in 2 out of 2 places. The bottom of the gate to memory care is broken and sharp to the touch. The bed for R13 is broken at the foot of the bed and is sharp which poses an immediate health, safety or personal rights risk to persons in care.
