What the inspector wrote

On 3/11/26 at 12:00pm Licensing Program Analyst (LPA) Kevin Gould conducted an case management deficiencies inspection to address deficiencies observed during an unrelated complaint investigation. LPA met with staff member Steven Bush who assisted LPA with obtaining records and documentation. LPA review the current staff roster and schedule. Upon review, LPA observed 10 staff members with criminal record clearances that have not been associated to the facility and two staff members without criminal record clearance (see LIC 421BG for individuals identified as un-associated or without criminal record clearance. Per the California Code of Regulations, Title 22, the following deficiencies are cited. Immediate civil penalties were generated as a result of the observed deficiencies. Exit interview conducted and a copy of this report and appeal rights were left at the facility.

    All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1569.17(b) shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 87355(c) this requirement was not met as evidenced by LPA review of staff records and observed two staff member without a criminal record clearance which poses an immediate health, safety or personal rights risk to residents in care.
