What the inspector wrote

On 11/17/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA initially met with one of the staff on duty and stated the purpose of the visit. The Administrator, Kathrine Caprio (AD), and Licensee, Delia Quinley, arrived shortly after. Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 elderly residents. Facility is cleared to accept up to 4 non-ambulatory. Facility is approved to retain/accept up to 2 residents receiving hospice services. Facility has fire door. Initial Observation: Upon arrival LPA observed 3 residents with 2 staff on duty. LPA observed required posters and facility license at the entrance. Room temperature was at 70 degrees Fahrenheit upon arrival. Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 4 resident bedrooms and the 2 bathrooms. LPA measured the hot water temperature in the 1 of 2 bathrooms to be at 119 degrees Fahrenheit. Resident bathrooms were observed to be in clean and good repair at this time. Fire extinguisher was observed in the living area and was last inspected on 8/14/2025. Smoke and carbon monoxide detectors were observed throughout. LPA observed centrally stored medications and sharp objects to be locked and not accessible to residents. {1 of 2} However, cleaning supplies, laundry soaps, and air freshener sprays were accessible to residents - cleaning supplies were observed under the bathroom sink; laundry detergents and additional cleaning supplies were observed in a garage cabinet; and air freshener sprays were observed in a linen cabinet. In the kitchen area, LPA observed sufficient seven day non-perishable and two day perishable food supplies. Kitchen refrigerator and freezer were maintained at regulatory temperature. The garage houses additional food supplies and chemicals. Outdoor area was inspected. LPA observed outdoor furniture for resident use. Ramps were observed to be in good repair at this time. Emergency walkways were observed to be unobstructed. Fence and gate were in good repair. Record Reviews: Review of 2 of 4 resident files was conducted, included review of Admission Agreement, Physician Reports, Needs and Services Plan, Centrally Stored Medication Record and Ambulatory Status. Resident cash resources were counted for 2 residents. Medication were reviewed for 2 residents. No issues were noted at this time. Review of 3 staff files, included review of background clearance, First Aid/CPR certificate, Health Screen, Initial and Ongoing Training. No issues were noted at this time. LPA also reviewed fire drill/disaster drill records; facility conducts monthly drill. Documents Requested: LPA requested a copy of current Liability Insurance Certificate, Surety Bond, LIC500, and LIC308 to be emailed to arvin.villanueva@dss.ca.gov. Per the California Code of Regulations, Title 22, Division 6, Chapter 8, deficiencies were cited. Exit interview was conducted with AD. A copy of the report was provided upon exit. {2 of 2}
