What the inspector wrote

Licensing Program Analysts (LPA) Cynthia Tamayo made an unannounced inspection to the Legacy Lane RCFE on 6/13/25 at 8:58 AM to conduct a Case Management Deficiency visit. LPA Tamayo met with staff, Isikeli Tuikenatabua and explained the purpose of the visit. Tamayo conducted an in-person interview with Cleaopatra Gardiner (S1) on 3/13/25 at 9:00 A.M. S1 informed LPAs that resident, Cal Sisommout (R2) died. LPAs conducted a record review and found that no incident report was submitted to CCL. No Death Report on file. S1 stated that a Death Report was not submitted because R2 was transported to hospital via hospice and died at the hospital. S1 stated they did not know she had to report the death to Community Care Licensing. This facility is hereby cited per Title 22 . An exit interview was held with Tuikenatabua . Appeal rights and a copy of this report were left at the facility.
