What the inspector wrote

On 10/15/25 at 9:30am Licensing Program Analyst (LPA) Kevin Gould arrived at Silver Springs RCFE 1 for the purpose of conducting a required 1 year annual inspection. LPA met with Administrator, Janssen Germono and together conducted a tour of the home. LPA and Administrator evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture LPA noted the lighting in the kitchen and above the laundry area are not operable at the time of inspection. LPA measured the water temperature, temperature measured at 131 degrees F which does not meet the 105-120 degree Fahrenheit regulation. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications secure from residents. LPA reviewed staff records and observed three staff members do not their criminal record clearance associated to the facility. LPA observed incomplete documentation for medication administration. LPA could not review all staff files and confirm health screening for all staff members as not all staff records were present at the facility for LPA to review. Per California Code of Regulations, Title 22 the following deficiencies are cited during today's inspection. An immediate civil penalty was issued. An exit interview was conducted, a copy of this report and appeal rights were left at the facility.

    Based on LPAs testing of hot water, the licensee did not comply with the section cited above as LPA recorded a temperature of 131 degrees F. which poses an immediate health, safety or personal rights risk to persons in care.

  • Request a transfer of criminal record clearance

    Based on staff record review, the licensee did not comply with the section cited above in 3 out of 12 staff members did not have appropriate documentation criminal record clearance or associated to the facility. which poses an immediate health, safety or personal rights risk to persons in care.

    Based on Staff record review, the licensee did not comply with the section cited above as the facility did not maintain records for all staff members listed on the schedule which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on staff records review, the licensee did not comply with the section cited above in 1 of the 8 staff files reviewed which poses/posed a potential health, safety or personal rights risk to persons in care.

  • Arrange appropriate medical and dental care

    Based on LPA review of medication administration records for all residents, the licensee did not comply with the section cited above as LPA observed missed documentation for all residents on 10/10 and 10/15 which poses/posed a potential health, safety or personal rights risk to persons in care.
