What the inspector wrote

On 11/13/25 at 2:10pm Licensing Program Analyst (LPA) Kevin Gould conducted a Plan of Correction (POC) inspection at Silver Springs RCFE 1. LPA conducted an inspection and review of previously cited deficiencies. LPA observed all staff have been associated to the facility. LPA observed appropriate lighting and operable light fixtures. LPA observed all staff training is up to date and meeting regulations. LPA observed the hot water temperature still exceeds regulations as LPA tested the hot water and recorded a temperature of 133 degrees F. LPA could not verify all staff members have a health screening/TB clearance. LPA observed staff members who administer medications are not documenting when medications are administered to residents at the time the medications are administered. POC clearance letters generated. Per California Code of regulations, Title 22, all deficiencies that had no been corrected have been cited again during todays inspection. A copy of this report and appeal rights were provided.

    The licensee shall arrange, or assist in arranging, for medical and dental care appropriate to the conditions and needs of residents. This requirement is not met as evidenced by LPA review or medication administration records which showed no documented medication administration for any residents on 11/13/15 which poses a poential health, safety and personal rights risk to residents in care.

  • Provide resident hot water for personal care

    Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). this requirement was not met as evidenced by LPAs testing of water temp to be 133 degrees which poses an immediate health, safety adn personal rights risk to residents in care.

    Maintain required personnel records for staff

    The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information such as health screening. This requirement was not met as evidenced by incomplete staff files reviewed which poses a potential health, safety or persona rights risk to residents in care.
