On 12/05/2025, Licensing Program Analyst (LPA) Christina Valerio arrived to conduct an annual required inspection. LPA rang the doorbell twice but no one answered. LPA contacted the facility number and was unsuccessful in reaching anyone. LPA Valerio contacted Licensee Crisel Centeno via cell. Licensee stated they do not have residents in care at this time. LPA scheduled a time to complete the annual at a later time. On 12/09/2025, LPA Valerio met with Licensee Orlando Carpio. LPA Valerio and Licensee toured the facility. LPA observed 0 residents in care. The facility is waiting to be vendorized by Alta California Regional Center. The facility physical plant was observed to be clean and in good repair. No health or safety concerns were observed. Due to zero residents in care, there are no staff besides the licensee. Licensee Orlando stated they would like to change the Administrator from Emily to Orlando. LPA Valerio requested the documents be sent in for review prior to making the change. No deficiencies were cited today. An exit interview was held, and a copy of the report was provided.
