What the inspector wrote

On 04/15/2026, Licensing Program Analyst (LPA) Kimberly Viarella arrived unannounced to conduct a case management visit regarding the revocation of this facility's license, and its closure plan.  LPA identified herself upon arrival, stated the purpose of the visit and asked to meet with the Designated Facility Administrator, Maria Caldeira. The Administrator was not at the facility at the time of this LPA's arrival. Staff contacted them and Caldeira and the Licensee, Ericson Sorrano arrived shortly after. LPA reviewed the Health and Safety regulation 1569.682 regarding forfeiture of license with both Caldeira and Sorrano line by line to ensure understanding and provided copies to both individuals. LPA discussed the components of a closure plan. Sorrano stated that they anticipated being able to submit a closure plan by 04/29/26 and that he thought that they could have the transition evaluations and updated care plans completed within the 2 weeks. Sorrano also stated that they would submit a draft of the eviction letter by the end of next week on or about 04/24/26. This LPA also conducted a walkthrough of the facility and observed one caregiver cleaning the kitchen after lunch, 1 resident sleeping, 1 watching TV in the common room, another caregiver assisting a resident in their room, and two other residents napping after lunch. There were 5 residents in care and 2 staff on duty. LPA provided technical assistance regarding the posting of Community Care Licensing reports. LPA stated that Health and Safety Code regulation 1569.38 required that the that the Accusation be "posted in a conspicuous place." Licensee posted the document prior to this LPA completing her visit. According the to the California Code of Regulations, Title 22, no deficiencies were observed or cited during today's visit.
