What the inspector wrote

Announced Office meeting, conducted via Microsoft Teams Meeting, was held on 04/22/26 with the following participants at this time: Stephenie Doub, Regional Office Manager Stephen Richardson, Licensing Program Manager Kimberly Viarella, Licensing Program Analyst Ericson Serrano, Licensee Maria Caldeira, Administrator The purpose of this office meeting was to go over the Decision and Order that was ordered on 04/13/26 and becomes effective on 06/17/26. All of the sections were reviewed with all of the above participants along with all applicable Title 22 Rules and Regulations, Health and Safety Codes, and Government Codes. In summary, the Order states that Ericson Serrano, the current Licensee has been excluded for a period of 2 years from the effective date of the Order. The Order further states that Eric L Serrano, Lilibeth E. Meza and Ericson E. Serrano have also been excluded unless they successfully petition for reinstatement. This petition may be submitted to the Department up to 1 year after the effective date of the decision. The Order also states that along with not working in any facilities licensed by the state of California, none of the above, including Ericson Serrano, may own a beneficial ownership interest of 10% or more in a licensed facility, or be an administrator, officer, director, member, or manager of a licensee or entity controlling a licensee. Going forward: 1. Community Care Licensing will: LPA Viarella will be providing the Closure Roster to the Licensee and Administrator by the close of business 04/22/26. Conduct weekly visits to monitor progress. Provide technical assistance as needed. 2. The facility will: Provide drafts of the Notice of Eviction and the Closure Plan by the close of business 04/22/26. Provide an approved Notice of Eviction to residents and their responsible parties by Thursday, 04/23/26. Complete updated assessments for all residents by Friday, 04/24/26. Notify LPA Kimberly Viarella when these assessments have been completed or if there were barriers to meeting this deadline. Prepare Closure Roster for LPA's visit next week. A scanned copy of this report will be emailed to the facility Licensee, Ericson Serrano, so it may be scanned, signed, and returned to this LPA via email at CCLASCPSacramentoSouthRO@dss.ca.gov.
