Pre-licensing visit·April 3, 2025

Licensing Program Analysts (LPA) Holly Williams arrived unannounced to conduct a follow up pre licensing visit. LPA Williams met with facility administrator Lovelie Arcega and explained the purpose of the visit. At the previous pre-licensing visit on 3/13/25 there were some deficiencies as listed below and the Applicant has corrected the deficiencies. Shed in backyard - Hole covered up and a locked gate installed. Storage in Room 1 - Applicant cleaned out storage. Bedridden room - has a non-ambulatory resident in it and the room is no longer a bedridden room. Based on a review of this facility during this Pre-licensing visit, it was determined that this facility was found to be in compliance at this time. Applicant has completed Component III. LPA Williams will notify the Central Application Bureau (CAB) that the pre-licensing has been completed.. An exit interview was held and a copy of the report was provided to Arcega.
