Pre-licensing visit·December 2, 2025

On 12/02/2025, Licensing Program Analyst (LPA) Pang Lee arrived announced to conduct a Pre-Licensing Inspection follow-up of the facility to ensure compliance with Title 22 regulations. LPA Lee met with applicant Saima Ali who assisted in today’s inspection. Saima Ali will be the administrator of this facility. Administrator certificate number # is 6072293740 and will expire on 10/20/2026. The facility has a fire clearance for 6 non-ambulatory residents to accept and retain at any given time. The facility will not employ staff who are live-in and will ensure that there are care in place for 24/7 for the residents. There were no residents in care at this time. LPA Lee informed applicant that the physical plant cannot be changed without notifying the department and that the submitted facility sketch needs to be adhere at all times. LPA Lee toured the facility with applicant Saima and observed no health and safety concerns. During today's visit, the following corrections were completed: • Applicant installed handrails on the front entry ramp. • Applicant leveled the step and added a ramp between the dining area and the common area to allow easy accessible passage. • Applicant posted Pub 475, If You See Something Say Something in the required size of 20" x 26". LPA Lee reviewed Component 3 with the applicant. The applicant has passed the pre-licensing component of the application process. LPA Lee will notify the CAB that the pre-licensing has been completed and passed. An exit interview was conducted with the applicant Saima, and a copy of this report was given to the applicant.
