What the inspector wrote

Licensing Program Analyst (LPA) Christina Valerio arrived announced to conduct a pre-licensing inspection. The facility was previously licensed and undergoing a change of ownership. LPA Valerio met with Administrator Grant and Licensee Aurora Maigue. They have an approved Dementia Care Program on file. LPA Valerio reviewed their Emergency Diaster Plan. Administrator Grant showed LPA copies of the Program Plan, which include the Dementia Care Plan, Hospice Care Waiver for size (6) residents, and Program Plan to care for non-ambulatory, ambulatory, and bedridden residents. LPA Valerio inspected the physical plant to ensure compliance with Title regulations. The facility currently has 6 residents in care. LPA Valerio spoke to 3 out of 6 residents. Resident bedrooms were observed to be fully furnished, free from odors, and with no obstructions to exit doors. Resident bathrooms were fully stocked. The hot water was measured at 119.5 degree F. Licensee was informed to keep tabs on the hot water temperature to avoid it from going above 120.0 degree. The facility had an adequate food supply. No health or safety concerns. Emergency Drill was conducted with staff during January of 2026. LPA Valerio observed the backyard, which had a old washer and dryer and a butcher meat slicer machine sitting on the side. Licensee stated previous licensee will remove by 01/31/2026. LPA confirmed this with previous Licensee Evelyn. LPA Valerio observed a storage room in the backyard and observed it to be used only for storage. Component III was conducted with the Licensee. Pre-licensing is complete and the facility has no deficiencies.Per California Code of Regulations (CCR) - No deficiencies were observed. An exit interview was held, and a copy of this report was provided.
