Pre-licensing visit·January 7, 2026

On January 07, 2026 at 9:50 AM, Licensing Program Analyst (LPA) Avelina Martinez arrived announced to conducted a Pre-Licensing Inspection of the facility to ensure compliance with Title 22 regulations. LPA Avelina Martinez met with Ketan Patal, who assisted LPA Martinez in today’s inspection. Facility has a fire clearance for six non-ambulatory residents. Ketan Patal will be the designated Administrator of this facility and has current Administrator Certificate and expires on August 19, 2027. LPA Martinez inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, and kitchen. The facility was clean, sanitary, and in good repair. The facility maintained a comfortable temperature for residents. The facility temperature was 70 degrees. The facility’s hot water temperature measured at 118 degrees. The facility’s fire extinguishers were last inspected in December 2025. The facility has an adequate food supply and has a food and emergency kit. Resident bedrooms and bathrooms are furnished and sanitary. The facility common rooms are furnished and sanitary. The facility has a designated area for activities. The facility has a medication room, which is made inaccessible to clients in care. The facility has a first aid kit, and facility files are maintained. The facility has implemented an electronic medication administration record software program. The exterior of the facility has a covered furnished patio for resident use. The exterior emergency exit gate is in good repair, and the exterior of the facility is clear of debris. The applicant has passed the pre-licensing component of the application process. LPA Martinez will notify the Central Application Bureau (CAB) that the pre-licensing has been completed and passed. Component III was discussed with the applicant. An exit Interview was conducted, and a copy of this report was provided to the facility.
