What the inspector wrote

On 11/19/2025 at 9:30 AM Licensing Program Analyst (LPA) Shakaricka Hughes conducted a visit to the facility. The purpose of this visit was to conduct a case management visit regarding a Special Incident Report (SIR) that was received by CCLD on 11/13/2025 concerning a resident that eloped from the facility. LPA met with facility staff Dede Nawasa. LPA explained the purpose of the visit. The current census is 3 with 1 facility staff. LPA interviewed facility staff (S1) regarding an incident of elopement on 11/11/2025. Based on interview with S1 it was revealed that the facility staff fell asleep, and was not aware of the resident eloping from the facility until the morning of 11/11/2025. Facility staff stated that law enforcement was called, and the resident (R1) was located by law enforcement on 11/12/2025 at 3:00 AM. LPA toured the facility, and noticed the facility has auditory signals on the front and back door of the facility. LPA reviewed the LIC 602 Physician's Report for resident (R1) and it was revealed that the resident is unable to leave the facility unassisted. Based on today's case management, a citation is issued under Title 22, Division 6. An immediate civil penalty in the amount of $500 is issued for the date of 11/11/2025 due to absence of supervision. An exit interview was conducted with facility staff Dede Nawasa. A copy of this report LIC 809, LIC 809-D, LIC 421IM and appeal rights was provided at the end of the visit.

The facility did not provide adequate care and supervision which resulted in R1 eloping from the facility on 11/10/2025 and returning to the facility on 11/12/2025. This posed an immediate health and safety risk to residents in care.
