What the inspector wrote

On 12/8/2025 Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced visit to the facility. The purpose of this visit was to follow-up with the licensee regarding a Plan of Correction (POC) cited during a visit to the facility on 11/21/2025. LPA spoke with facility staff/ administrator Dede Nawasa. The current census is 4 with 1 facility staff. LPA spoke with the licensee regarding the status of the Plan of Correction cited Title 22 regulation 87468.2(a)(4) Additional Personal Rights of Residents in Privately Operated Facilities. The licensee stated that the facility is in progress of completing the training related to resident care and supervision and will send LPA proof of training for all staff currently working in the facility by 12/12/2025. No citations were issued during this visit. A copy of the report was provided.
