What the inspector wrote

On 03-19-2026, Licensing Program Analyst, Arvin Villanueva (LPA), arrived announced to conduct the pre-licensing inspection visit. LPA met with applicant, Goutam Rattu, and stated the purpose of the visit. Overview : Facility is a one-story home located in a residential neighborhood. Facility will be licensed to serve up to 6 elderly residents. Facility is fire cleared for up to 6 non-ambulatory residents. Facility does not have fire clearance for bedridden, delayed egress and locked exteriors/interiors. Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. Outdoor Area: Front yard was observed to be maintained and clear of debris and obstruction. The back area has outdoor furniture. There is pergola going to the back patio that Goutam has stated will be covered. Outdoor passageways, walkways, driveways, and ramps are free from obstructions. Fence and gate are in fair condition at this time. Advisory provided to Goutam that fence may require non-immediate repair. No pool or bodies of water at this time. Common areas: Living room and dining room were observed to be in good repair. Fireplace is covered. Kitchen was observed to be clean. The garage will be locked as it will be a storage area for other dangerous items. There is a fire door leading to the residents’ and staff bedrooms. Advisory was provided to Goutam to ensure fire door remains close at all times. Residents' Bedrooms: LPA inspected 3 of 3 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. Each bedroom is equipped with smoke detector. Bedroom #1 is cleared for staff bedroom. Bedroom #4 has a exit door for all residents and staff for emergency evacuation. Bathroom: Facility has 2 bathrooms. Bathrooms have working toilets, wash basins and full baths with showers. Bathrooms are equipped with non-skid flooring. Grab bars were in good condition. One of the bathroom is located in bedroom #4. {1 of 2} Linens & Hygiene Supplies: Adequate supply of linen and hygiene supplies were observed. Emergency and Exit Plan : Emergency Disaster Plan will be posted & available for review. One fully charged fire extinguisher was observed in the kitchen. LPA requested Goutam to update his facility sketch and Emergency and Disaster Plan to reflect the location of the gas shut off valve. Dangerous Items: Sharp objects, cleaning supplies, toxins, and chemicals were locked. Advisory was provided to ensure all dangerous items are always locked and not accessible to residents in care. Water and room temperature: Hot water temperature was taken in two bathrooms and measured at 117 degrees Fahrenheit. Room temperature was maintained at 68 degrees Fahrenheit throughout the visit. Food Service: Dishes, cups and flatware are stored in the kitchen cupboards, inspected and in good repair. Knives, cutlery and other sharps are locked in cabinet under the sink. Smoke Detectors and carbon monoxide: Smoke detectors were observed in each bedroom, common areas and hallways. One was tested and found to be working. Carbon monoxide detectors were also observed. One was tested and found to be working. Appliances : Stove burners and oven are in good working condition. Refrigerator/freezer temperature were within regulatory standard. Washer and dryer are located in the garage. Medications, First-Aid Kit & Records: First aid kit has been inspected which has at least the following: thermometer, tweezers, scissors, antiseptic, bandages, gauze and manual which are stored in locked cabinet in the kitchen/dining area, available for staff use but inaccessible to residents. There is a medication closet located right before the fire door. Residents & Staff Files: Applicants will not be handling cash resources of residents at this time. Activity Supplies/Materials: LPA observed activity supplies stored in the dining area. Advisory was provided for licensee to provide residents with a variety of activities and choices. Component III: Component III was conducted with applicant, Goutam Rattu. This pre-licensing is complete and no deficiencies were observed. Goutam was advised to contact the office once he admits his first resident. An exit interview was conducted, and a copy of the report was provided. {2 of 2}
