Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection. LPA met with caregivers, Shavanna Reid and Opal Wright. Administrator, Nicoleta Hegseth, arrived at 3:00 pm. LPA observed (2) residents in the common area and (3) residents in their rooms. The facility is licensed for (6) non-ambulatory residents. Currently, there are (0) residents under hospice care. LPA and the Administrator toured the interior and exterior of the facility including the common areas, (2) private resident bedrooms, (2) shared resident bedrooms with a bathrooms each, (1) separate bath with a walk-in shower, kitchen, staff room, locked laundry area and garage. LPA observed the facility to be clean, in good repair and odor-free, and the bathrooms have the necessary grab bars, skid-resistant flooring and hygiene supplies. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps, daily medications and toxins are locked in the kitchen. Additional medications are secured in the laundry room. There are sufficient linens/blankets/incontinent/PPE supplies and night-lights throughout. The inside temperature was 76*F, and hot water measured 114*F in a resident bathroom. Fire extinguisher last serviced 9/5/2025, and smoke/monoxide alarms are functioning. There is a large backyard area/patio with covered seating and an unlocked gate. There are no pools. All required postings are posted are visible in the common areas. There are games, puzzles and activities in the common area. LPA reviewed (2) resident files and (6) staff files. Files were organized, complete and contained current paperwork. Medications were reviewed for (1) resident- orders match medications. Staff are completing required initial/annual training, including First Aid/CPR. Administrator RCFE Certificate #7024645740 (exp 2/15/27). There were no deficiencies observed and no citations issued. Exit interview with authorized staff. Copy of report provided.
