What the inspector wrote

Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with caregiver, Isaiah Davis, to conduct a health and safety check. During visit, LPA attempted an interview with resident (R1) and reviewed documentation for R1. LPA also interview staff (S1) and toured the premises. Per R1's Medical Assessment dated February 13, 2026, R1 requires a wheelchair and hoyer lift for mobility, transfer, and repositioning. LPA did not observe a hoyer lift or wheelchair for R1 to use on the premises. S1 stated that R1 also discussed not feeling safe with current bed rails. As a result of today's inspection, a deficiency is being cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Deficiency is listed on 809-D. Exit interview was conducted. A copy of this report and appeal rights were provided. Signature on these forms acknowledges receipt of these documents.
